Annual Report

Each year, public libraries in Alberta are required by the Libraries Act to complete the Public Library Survey and Annual Report of Public Libraries in Alberta form produced by the Public Library Services Branch of Municipal Affairs. This form is commonly refered to as the "Annual Report."

The form is divided into two parts:

1. Public Library Survey: consists of Sections A to C and includes public library identification information and questions about current board members, meeting dates, service points, library hours and plan of service.This information is used to compile a provincial directory of public libraries, to produce mailing lists and to determine compliance with provincial library legislation and regulations.  

2. Annual Report of Public Libraries in Alberta: begins at Section D and extends through the remainder of the form. The questions under the Annual Report capture public library activity during the previous year. This report documents the service activity of public library boards in Alberta and provides information on performance of our public libraries. Data on specific performance measures will be collected to produce Public Library Statistics for the province of Alberta.
 

New in 2011:

The 2011 grant application forms are now available on the Public Library Services Branch website at http://www.albertalibraries.ca/grant-information. These forms include:

•  2011 Annual Survey and 2010 Annual Report of Public Libraries
•  2010 Statement of Receipts and Disbursements
•  2011 Budget
•  2011 Application for Financial Assistance

Starting in 2011, the data for the Survey and Annual Report will be collected using a new online tool called LibPAS. All library managers and/or designated respondents should have received an email from the PLSB with complete login information to access the Survey/Annual Report form in LibPAS. Please be sure to keep this information secure.

In order to ease the transition to using LibPAS to fill in the Survey/Annual Report, PLSB has created several support documents for your use. The Companion Guide for filling in the Survey and Annual Report has been updated and now has an Appendix containing screenshots of the Survey/Annual Report as it appears in LibPAS. There is also a document with instructions on how to use LibPAS and an FAQ page (this page will be frequently updated as new questions come in to PLSB). Please be sure to review these documents/pages carefully before logging in to complete the Survey/Annual Report.

The deadlines for submitting these documents are as follows:

  • 2011 Annual Survey/2010 Annual Report of Public Libraries - February 28, 2011; Library boards using LibPAS have until March 31, 2011 to complete and submit the report
     
  •  Grant application (including the 2010 Statement of Receipts and Disbursements and 2011 Budget) - June 15, 2011. Please be sure to get your documentation in on time. Failure to do so will result in significant delays in the processing of your library board's grant application.
     

Survey Templates

Part of the Annual Report involves surveying library users to see how satisfied they are with the services provided by your library.

You are also required to conduct a computer use survey to determine how often your library's computers are being used by the public.
